Workload Transfer Form

One of the many roles of Gwent LMC is to ensure that primary care are not undertaking unfunded work from secondary care. We hear about issues from practices and if there are recurrent themes then we escalate them at our LMC meetings or our liaison meetings held between ourselves and the Health Board Contracting Team. We also attend meetings with the Divisional Directors of the Healthboard on our practice's behalf to discuss recurrent themes of interface issues. 

 

In 2018 the All Wales Communication Standards were published which set out the expectations of the interface between primary and secondary care:

We have revamped the way in which practices can raise concerns to us about breeches of the All-Wales Communication Standards. We have designed a Microsoft Form for GPs to fill in and it has the ability for you to add a document which is good because the Healthboard will often ask us for documentary evidence when we raise recurrent themes.

It is absolutely imperative that any letters uploaded to our portal have all patient identifiable information removed, apart from the NHS number in order to protect patient confidentiality.
